About the Louisiana Donor Registry and Louisiana Organ Procurement Agency

The Louisiana Donor Registry – (registry.donatelifela.org) –is a donor registry, as set forth in the Louisiana Anatomical Gift Act, La. R.S. 17:2351 et seq. As such, it is a statewide database of documents of gift, and any amendments or revocations thereof, of Louisiana residents or individuals residing in this state that can be accessed after a donor’s death so that donor decisions regarding organ, eye and tissue donation can be honored.

While this Site is mainly intended for users in the United States, we understand that some of our Site users may reside elsewhere. If you are located outside of the United States, please note that by using our services and our Site, you may be transferring your personal information to a country that does not have the same data protection laws as your home country.

Personally Identifiable Information: You can visit this Site without providing us with any personally identifiable information (“personal information”). However, we collect personal information from you when you voluntarily register to be an organ, eye, and tissue donor. The personal information we collect includes such items as your name, address, email address, gender, date of birth and last four (4) digits of your social security number. For those who voluntarily register to be a donor with the Louisiana Office of Motor Vehicles (OMV), the OMV provides us with identification information from your driver’s license.

Email Information: During the course of registration, LOPA will send you emails(s) and you may send email(s) to LOPA, which tracks the emails and associated information.

Site Usage Information. Whenever you use this Site, certain types of internet usage information may be received and stored, such as information from your browser, including but not limited to your IP address, pages visited, the length of time you spend on the Site, type of browser software and operating system you are using, internet domain through which you access the internet and files you download, which are recorded in the Site server logs. However, no personal information is collected during the process.

We use the registration information that you provide to honor your decision should the potential for donation arise. An authorized organ, eye or tissue recovery staff member of LOPA, or one of the eye banks located in Louisiana, will review your information to affirm your registration and present your decision to donate to your legal next of kin.

We may also use your information to: (a) contact individuals as allowed under Louisiana law; (b) verify or confirm the registration of a donor; (c) create a legal document of gift for a donor or prospective donor; and (d) add, remove or modify an individual’s registration record with their expressed consent.

Our Disclosure of Information That We Collect

We consider your personal information to be yours and only yours. Therefore, we will not sell or provide your personal information to third parties, including third party advertisers, except as specifically described herein. Your information may only be disclosed: (a) to LOPA or authorized organ, eye and tissue recovery organizations and staff in order to honor your donation decision; (b) if you request LOPA to do so; (c) when necessary to establish or exercise LOPA’s legal rights or defend against legal action; or (d) as otherwise required by law.

Steps We Take to Protect Your Information

The security of your personal information is important to us. In order to prevent any unauthorized access or disclosure of the personal information that we collect, LOPA maintains reasonable administrative, technical and physical safeguards designed to protect the personal information that you provide on this Site. Unfortunately, no method of transmitting or storing data is impenetrable and LOPA cannot guarantee the security of this Site nor can LOPA guarantee that the information you supply will not be intercepted while being transmitted over the Internet. By using this Site you acknowledge that LOPA is not responsible for the actions or illegal acts of third parties.

When you submit your donor registration form, you will receive an email from LOPA with instructions on how to create a user account and password, which you may subsequently use to view, amend or revoke your document of gift. By accessing and using a user account, you agree to maintain the confidentiality of the username and password you select.
